Download the form, fill the form completely, sign it, and fax it back to us.
2) Fax us a copy of your original 'Business License'.
3) Fax us a copy of your original 'Resale Tax Certificate'.
4) Our representative will contact you, verify your information, and approve you as a new distributor.
